Leave of Absence
Any situation in which an employee must take time away from the company. This can include, without limitation, the Family and Medical Leave Act (FMLA), the Paid Family and Medical Leave Act (PFMLA or PFML), Intermittent FMLA (IFMLA), medical, personal, or any other leaves protected or unprotected that the company may offer. These are managed and reviewed by our People and Culture team. Leave of Absence requests require a start date and, preferably, an end date, along with your doctor's note or recommendation. Make sure to have this information ready, as this will be reviewed by our People and Culture team.
Accommodation Request
A request by an employee that they cannot perform a specific aspect of their job due to a medical condition. If you have a doctor's note or certification, you may send in your request to careteam@intelycare.com, and we will forward it to our People and Culture team.
Unemployment Claims:
Prior employees can seek unemployment benefits. Be ready with the following information below

- Case ID/ Reference Number
