The information listed on this page is taken directly from the Medically Home orientation and serves as a reminder if you have already been cleared to work these shifts. If you are interested in accepting Medically Home shifts, please reach out to 617-812-9084, Monday-Friday, 9am-5pm EST.
Remember these are acute care patients. If they start to decline during your visit, immediately contact the command center through the PERS device for emergencies or through the phone for non-urgent issues.
For CNAs, ensure that the next aide has arrived prior to leaving if the patient is getting continuous aide services.
Accepting a Medically Home Shift
- To start, accept the shift through the IntelyCare app.
- DO NOT WORK ANY SHIFTS THAT ARE NOT LISTED IN THE INTELYCARE APP. If the MHG team reaches out to you directly to ask you to work a shift that is not in the app, please ask them to put it in the portal before agreeing to work. Any adjustments to your shift should be changed by the MHG service coordinators.
- Once you have accepted the shift, a secure email will be sent to your email address with a link to a secure site for patient information.
- The email will contain important information like patient demographics, code status, and any relevant precautions.
- If you do not see the email, make sure to check your spam and trash folders.
- DO NOT TAKE ANY SCREENSHOTS OR SHARE THE INFORMATION IN THE SECURE EMAIL.
- If you have questions, call the Command Center and talk to the service coordinator. They are responsible for scheduling. The IntelyCare support team does not have any details on the patients you will be treated, as this is confidential medical information.
Safety Requirements
- Facemasks must be worn at all times by all parties. This includes the patient.
- The patient may choose to wear a cloth mask.
- Regularly wash your hands with hand sanitizer.
- Whenever possible, keep 6 feet physical distance from the patient and others in the home.
- Limit home entry to essential caregivers only, those performing pickup/drop-off services will leave items in the main entrance whenever possible.
- Only essential people should be in the room during the treatment/service session.
- When putting on PPE, follow the order below:
- Gown > Mask > Eye/Face Protection > Gloves
- When removing PPE, follow the order below:
- Gown > Gloves > Eye/Face Protection > Mask
- All used PPE should be placed in your trash bag and left in the outside trash bins. Do not leave in the patient's indoor trash bins or the red biohazard waste container.

Shift Revision and Meal Break Revision
