The required documents to work with IntelyCare vary by state, but some documents are the same across the board. Completing these documents promptly will help speed up your account activation, allowing you to start your journey in providing quality care as soon as possible.
IMPORTANT: Uploaded document images are now hidden. Previously, an image of the uploaded document would be displayed; now, a message will be displayed, instructing you to contact support if you need a copy of the specific document.
Required Onboarding Documents
IntelyCare does not cover the cost of a TB test and a physical exam. You'll need to obtain it at your nearest clinic or schedule a doctor's appointment.

A 2-step TB test from within the last year for PA Only (other States only require a 1-step TB test).
- A chest x-ray can be substituted for this requirement when recommended by a doctor due to prior infection or vaccination against TB, along with the completed TB Questionnaire.

A physical exam from within the last year.
- PA requires a physical exam to indicate that you are free from communicable diseases and can perform your work duties without restriction.

CPR certification within 2 years. All CPR cards must include in-person training and must be from the American Heart Association (AHA) or the American Red Cross. ONLINE Only courses are NOT accepted.
- CPR is required for all Nurses.
- Mandatory for CNAs in FL, DE, and VA.
- ACLS card from AHA must be submitted along with a BLS card.
- A social security card, passport, or birth certificate.
- Valid photo ID in the form of a driver's license, student ID, or state ID.
- Work License
- Background Check Consent form
- State Police SP167 Form for VA applicants.

- BLS or Basic Life Support Certification (Facility Requirement)
- Covid Test (Facility Requirement) - There are two main COVID-19 test types: diagnostic and antibody tests. Diagnostic tests detect current infection with SARS-CoV-2, the virus causing COVID-19. Common diagnostic tests include: Molecular tests, such as polymerase chain reaction (PCR) and other nucleic acid amplification tests (NAATs) tests, which detect genetic material called RNA from the virus
Antigen tests, often referred to as rapid tests or, for some, at-home or self tests, which detect proteins called antigens from the virus. Antibody tests check your blood for antibodies made in response to the virus, usually collected by a healthcare professional via finger stick or vein blood draw.
