IntelyForms are optional forms in the app that all Intelypros can print and bring to their physician. App access is currently limited during onboarding.
How to Access Intelyforms from the app
- Log in to the App.
- Select the Menu bar.
- Select My Documents .
- In the top right-hand corner, select the Intelyforms tab.
- Choose the form and select the three dots to the right of that form.
- Choose the E-mail to email the form to yourself or the physician.
- If you click more options, you will see the available options to send the form to yourself or the physician.
- Once the form is completed by a doctor, take a picture and upload it to your profile.

Why does my drivers license keep coming back?? I send the photo of my license and another copy of my address form because it’s different than my drivers license
The onboarding steps in my app have locks on them. I can't upload any documents. What can I do? Joseph D Yates, LPN.

How can I obtain a copy of the health clearance form?
The directions that are listed above to obtain the medical clearance form cannot be located on the app, how do I obtain the medical form?
There is no option for "My documents" in the menu? Where am I supposed to locate the forms for TB and Physical Exam??
