Name Changes

Following a legal name change, you will be required to submit proof of your new name.  This is because our existing paperwork is under your prior name; to protect your identity, it's important that we have proof that your new paperwork actually belongs to you.

The following documentation is commonly accepted as proof of a legal name change:

• Marriage Certificates
• Divorce Paperwork
• Court Decrees of Name Changes

You should also submit an additional requirement to change your name for payroll as required by IRS.
